Terms of Site Use (fee for service - $2,000)

The user is required to sign-up and deposit ten percent ($200) of the evaluation before scheduling, at which time you will receive an email or phone call within a day to schedule the evaluation to take place as soon as a week later or further out if so desired.  Upon arrival, the remaining $1,800 will be processed on site prior to the evaluation.  The $200 deposit will go toward No show or late cancellations (48 hours) fee consistent with the CANCELLATION AND RESCHEDULING POLICY detailed below. These terms are subject to change at any time without notice. ​Please bear in mind that Dr. Montoya does not take insurance and is not on any insurance panels.  Should you elect to submit claim forms to your insurance company in an effort to pursue payment from your insurance company, Dr. Montoya will be happy to complete the HICFA form with all the needed details for you to submit to your insurance company directly. 

Cancellation & Rescheduling Policy

Rescheduling or cancellation after the appointment has been scheduled will result in a two-hundred-dollar ($200) fee being charged to the client's card and is not refundable. After the second instance of a cancellation or rescheduling, the fee will increase to four-hundred ($400) dollars and is non-refundable.  No-shows will result in full forfeiture of "deposit payment" for a scheduled appointment. However, this forfeiture can be used towards the rescheduling of the appointment for a later date, less the fee described above.

Please review the terms and conditions regarding cancellation, rescheduling, and no-shows. We strive to provide our clients with the best possible experience. Attempting to change a scheduled appointment will interfere with others who could have scheduled at that time. This is the reason for such stringent terms.  Please carefully consider the date and time before you commit to ensuring you receive the very best one-on-one evaluation with the absolute highest quality

Payment Option(s)

The neuropsychological evaluation is a flat fee of $2,000.00, which is required after agreeing to the terms and conditions contained herein to schedule an appointment. If the evaluation involves traumatic brain injury accompanied by voluminous records to review, additional fees for reviewing records may apply. Although we are not on any insurance panels, we will be happy to complete paperwork for you to submit to your insurance company so that you can go forward in pursuing reimbursement.
